FORMS FOR CPT STUDENTS

1. Internship Cooperation Agreement and Approval : Form to get approval from academic advisor and DSO for CPT eligibility.
2. Placement Letter Template :Template to be used by employer for student placement letter. It should be submitted by the student to the academic advisor after CPT eligibility is confirmed.
3. VIU Internship Site Registration Form : Form to be filled (online or print) by the employer, and to be submitted to the Career Center for validation of the placement organization.
4. Time sheet: to be filled by the site supervisor and submitted to the Faculty Advisor.
5. Intern Evaluation form : Evaluation of the student by the site supervisor.
6. Intern Evaluation of Internship Experience: Evaluation by the student of his/her internship experience.
